Office Policies
Office Visits Please bring a valid driver's license, current insurance card, and co-pay/payment at every office visit.
Preferred Provider We will attempt to schedule you with your preferred provider. We strive to provide continuity of care and follow our patients through the years in sickness and in health.
Prescription Refills Please bring all your medications to your routine visits. The doctor/nurse practitioner will write all refills at that time. If you are in need of a medication refill, we ask that you contact your pharmacy and have them notify us. You can request medication refills via the patient portal at any time. Please do not wait until you are out of your medicine to contact us, and allow 48 hours for all refills to be processed. We ask that you do not call our emergency service after hours for refills.
Referral Requests We will make every effort to have your referrals done before your specialist visit. We kindly ask that you give us at least 2-3 business days to process requests. You are responsible to pick up your referral. A referral will not be issued if you have not been seen by our doctor for the problem or we did not refer you to the specialist.
Test/Lab Reports Test results that require immediate attention and abnormal lab results will be promptly addressed by one of our providers. Normal results are not typically reported; however, you are welcome to call the office to request a copy of your results.
Missed Appointments As a courtesy, our staff, when possible, will call with appointment reminders. It is your obligation to keep your scheduled appointments. When possible, please provide us at least 24 hours notice if you are unable to keep your appointment. If you are late for your appointment (10-15 minutes), we will try our best to accommodate you. On certain days, however, it may be necessary to reschedule your appointment. We strive to minimize any wait time; however, emergencies do occur and will take priority over a scheduled visit. We appreciate your understanding and respect your time.
